Emergencies:

You can call at any time for emergencies.  If the office is closed, leave a message with your name, phone number (incl. area code) and the nature of the emergency and we will get back to you.  Current patients of Dr. Bloom's can look him up in Skokie and call him at home.  (Sorry, but new patients with an emergency can only be serviced during office hours.)

Cancellations:

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice.  This courtesy on your part will make it possible to give your appointment to another patient.  Patients may be billed for late cancellations and no-shows and we may opt to require prepayment to secure another appointment.  We really prefer you only schedule definite appointments.

Insurance:

We accept most insurance plans.  Patients are responsible for deductibles, co-payments, and remaining balances.  Talk to Kim regarding the option to receive a discount if you pay at the time of service and receive insurance reimbursements directly from the insurance company.

Financial Policy:

Deductibles, co-insurances and co-payments are due at the time of service.  Forms of payment accepted are cash, check, insurance assignment, any major credit card.  Unpaid balances beyond 30 days are charged interest or a minimum billing charge.  If you need to finance your treatment, talk to Kimbra about low-interest financing available through outside plans.
